Your Photos:
Part of the intake form requires you to take photos of various rooms within your house (“Your Photos”). Your Photos are handled in a commercially reasonable manner consistent with this policy and industry standards. Further, Your Photos are temporarily stored on a Service Provider’s server for analysis purposes only. Your Photos are then deleted upon the completion of the Service Provider’s analysis. Your Photos are never included in any report generated by the Service and Your Photos are deleted within a maximum of thirty (30) minutes from the time the analysis is complete by the applicable Service Provider.
Health and Mobility Information:
Where applicable, the intake form may require you to input certain health and mobility related information, such as describing certain fall and risk concerns, providing information about how mobile you are, and relaying health conditions you may have been diagnosed with (collectively, “Health and Mobility Information” and together with Your Photos, “Additional Data”). This Health and Mobility Information is handled in a commercially reasonable manner consistent with this policy and industry standards. This Health and Mobility Information travels with Your Photos to the Service Provider’s server, where it is stored temporarily, until the Service Provider completes the necessary analysis to generate the report. Once the report is generated, this Health and Mobility Information is deleted immediately, but no later than thirty (30) minutes from the time the Service Provider completes the analysis. The Service does not require you to submit any protected health information and thus you represent and warrant that any Health and Mobility Information submitted and inputted by you to the Service is not considered “protected health information”, as that term is defined under the Health Insurance Portability and Accountability Act of 1996.
We use the collected data for various purposes:
To carry out our obligations and enforce our rights arising from any contracts entered into, between you and us, or you and a third-party service provider
To fulfill any other purpose for which you provide it
Your information, including Personal Data, will mainly be processed in the United States, but some third-party providers may process it elsewhere.
